Cryptococcal Meningitis in Young, Immunocompetent Patients: A Single Center Retrospective Case Series and Review of the Literature

Abstract Background Cryptococcal meningitis is an uncommon but serious infection with a high mortality and morbidity. Classically described in immunocompromised patients, including those solid organ transplants or HIV/AIDS, cryptococcosis has also been reported young otherwise healthy albeit rarely. Methods We retrospectively searched for all cases of cryptococcal (≤50 years) previously patients no known immunocompromising conditions from January 2015 to 2022 at Indiana University Health (IU Health). Additionally, PubMed literature review was performed the keywords “cryptococcal meningitis” “immunocompetent” 1988 2022. Clinical courses, outcomes treatment regimens were evaluated. Results identified 4 local ≤50 years. Three due Cryptococcus neoformans one experiencing post-infectious inflammatory response syndrome (PIIRS). The search 51 additional 32 (63%) being caused by 8 (17%) gattii. Of cases, only two resulted death directly cryptococcosis. Fifteen (29%) had PIIRS steroid documented 11 15. Antifungal induction duration varied predominately consisted amphotericin flucytosine mean 5.0 weeks. Conclusions young, likely under-recognized. (akin IRIS observed HIV/AIDS) prolonged recovery should be concern. Determining risks factors these remains elusive.
